MEMBER CARD CREATION
As a member of the Case Student/Dependent Medical Plan, you can enter your network ID below to obtain a printable copy of your prescription card. You must have a printer available and attached to your computer to print the card on. This card can then be used at any participating pharmacy to obtain your prescriptions.
