All students participating in the Case Student Medical Plan are automatically enrolled in the Prescription Benefit program.
Dependents enrolled in the Case Optional Dependent Medical Plan are automatically enrolled in the Prescription Benefit program.
Completion of an Affidavit is necessary for the enrollment of a domestic partner. Forms are available at the University Health Service.
